We’re committed to serving patients inside - and outside - the walls of our healthcare clinics. Our patient outreach efforts aim to improve overall population health and reduce the barriers patients sometimes face on their road to wellness. Healthcare doesn’t stop at the door of a doctor’s office, and neither does our commitment to better patient health.

EMPOWERMENT FOR BETTER LIVING
These workshops will help empower you to take control of your health so
you can live a happier, healthier life. This chronic disease management
program was developed by Stanford University. These workshops are for
anyone with a chronic illness (diabetes, arthritis, high blood pressure,
heart disease, chronic pain, anxiety, etc.), family or friends of a person
with a chronic disease and anyone interested in learning how to live a
healthier life.
LINDSAY DIABETES PROJECT
The Lindsay Diabetes Project is a collaborative effort through Healthy Kids-Healthy Lindsay and a number of stakeholders that includes health care providers, community-based organizations, and community members. The project is funded through the Lindsay Hospital Board and the mission is to optimize the health of Lindsay residents affected by diabetes through a comprehensive, culturally sensitive, family-centered approach. This approach includes providing a 6-week educational series based on the Project Dulce curriculum conducted by bicultural/bilingual Promotoras, linking them to health/social services, physical activity opportunities and providing ongoing support through monthly support groups. In addition to this participants will have the opportunity to work with a Pharmacist to review their medications.
